Dr. William Mack, an oculoplastic surgeon in south Tampa with 29 years of experience, was recently a featured speaker at the Masters session during the 7th Annual Global Aesthetics Conference in Miami Beach, Florida. At this multidisciplinary conference, which features four aesthetic specialties, Dr. Mack’s presentation “Pearls for Cosmetic Blepharoplasty – What I’ve Learned Over the Past 28 Years” focused on his personal experience.
Dr. Mack noted in his presentation “When eye contact is made, we immediately begin forming our first impressions. Judging someone to be tired, sad, angry, or pleasant is based on the appearance of the eyes as this area is the most important aesthetic region of the face. Evolving trends in oculoplastic surgery include an emphasis on achieving natural aesthetic outcomes while avoiding overdone, unnatural results. The ultimate goal is to create a natural, more youthful, refreshed appearance through various cosmetic techniques. This goal is achieved by focusing on maintaining volume in the eyelids and avoiding the hollowed outcomes in eyelid surgery that were often seen in patients who underwent surgery in prior decades.”
“In the past, when patients underwent cosmetic procedures, the results were often noticeable and apparent that surgery had been performed. Today, we’re seeing a shift as more patients are opting for smaller, more subtle adjustments earlier on, before the signs of aging become too advanced. These more gradual tweaks allow them to maintain a natural look without waiting for more significant changes to occur. As a result, the outcome is a refreshing and natural appearance.”
“On the contrary, many celebrities have clearly had a significant number of fillers that have exaggerated their appearance that make them look overfilled and puffy. Instead of making subtle changes, these celebrities have made dramatic changes by using many fillers in one area. We’ve learned that focusing too much on enhancing just one or two features doesn’t necessarily improve someone’s appearance. Fortunately, people are moving away from this look. Now, the trend is shifting to achieve natural results where the focus is on lifting and repositioning areas that have sagged over time by combining multiple subtle procedures.
“It’s important to have an individualized treatment plan for each patient that focuses on their unique needs to achieve the best aesthetic outcome. The preoperative examination focuses on the face in its entirety including an emphasis on facial proportions. The examination of the upper eyelid includes an assessment of the height of the eyelid in relation to the pupil, the effect of the brow position on the eyelid height, and an analysis of the amount of excess skin. The lower eyelid assessment includes assessment of any evidence of hollowness (tear trough deformity), orbital fat herniation, and amount of excess skin with the surgical goal focusing on achieving a smooth transition in the lower eyelid/cheek junction.”
“In summary, rejuvenation goals in oculoplastic surgery focus on combining surgery, skin rejuvenation, and nonsurgical procedures such as fillers and botulinum treatments to enhance the patient’s appearance with a more youthful look. Trends in oculoplastic surgery include an emphasis on achieving natural aesthetic outcomes while avoiding overdone, unnatural results. The most common oculoplastic surgery, upper and lower eyelid blepharoplasty, can be combined with other cosmetic procedures, such as endoscopic brow lift, laser skin resurfacing, and facelift surgery. The goal of any facial cosmetic procedure is for the patient to be able to look in the mirror and notice a more rested, refreshed appearance that is natural.”
